Report a Concern

Is this an emergency? Do you believe a student or someone else may be in immediate danger?

  • Person is actively suicidal
  • Threats of violence to one's self or others
  • Medical emergency
  • Any other type of an emergency

If there is an emergency situation, please call University of Indianapolis Police at 911 then complete the appropriate referral form below as needed.

While staff will review the forms, they might not be reviewed immediately, which is why it is important that emergency situations are referred to UIndy Police.

Members of the campus community have the opportunity to report concerns related to student safety, needs, and overall wellbeing. Prior to submitting the Student of Concern form, it is requested that you connect with the student directly to discuss the concern and make them aware the form is being completed on their behalf.

Members of the campus community have the opportunity to report issues that they believe may be a violation of the Student Code of Conduct, found in the Student Handbook, including but not limited to on-campus use of alcohol, tobacco, or controlled substances, disorderly conduct, and dangerous or disruptive behaviors.

UIndy is committed to providing an educational and employment environment that is free from Sex Discrimination, Harassment, and Misconduct per the Title IX/Sexual Misconduct Policy. This form can be completed by either complainants, witnesses of violations, or mandatory reporters.

UIndy is committed to fostering a safe and respectful campus environment free of hazing per the Hazing Policy. This form can be completed by either targets of hazing-related incidents, witnesses of violations, or mandatory reporters.

UIndy is a welcoming and inclusive environment and is committed to providing a safe living, learning and working environment for all its community members. Actions and behavior that are aligned with ideologies that may impede equity and inclusivity are not tolerated by the University per the Notice of Non-Discrimination.This form can be completed by either targets of bias-related incidents or witnesses to them.

If you witnessed a crime on campus and would like to anonymously report it, please fill out this form with the most complete details possible. The form will be sent to the UIndy Police Department when you click submit. In the event of a sexual assault, all the information will be forwarded to the University’s Title IX Coordinator.
